Appointments
Parents and guardians are able to request an appointment for their child via our online form or by calling our office during open hours at (515) 222-1800. If for any reason you are unable to keep a scheduled appointment or will be delayed arriving, please call us as soon as possible. If it is your first appointment with us, please arrive early to complete necessary paperwork in advance.
Insurance and Billing
We accept most traditional insurance plans. Please contact our office to verify if we accept your plan. We are in-network providers of Delta Dental, Blue Dental, Principal, Cigna, Metlife, and Hawk-I. We do not participate in Health Management Organizations, however we will be happy to file your insurance claims for you. We will collect for services at the time they are completed and provide our best estimates for the portion that insurance may not cover. Please be aware of any deductibles or coinsurance that may be due at the time of your appointment.
Payment Options
We accept check, cash, and all major credit cards. We also offer flexible payment plans and Care Credit if financing is needed. Please ask about financing options for details.
